How to fill in the form

1. Before you begin, please download and complete the species list. This is because you will be required to upload it as part of this form.
2. If you have surveyed more than one hedge, complete the form below one at a time. Once you have filled it in for one hedge and submitted it, you will be asked on the next page whether you have another to submit. You may enter as many as you have surveyed.
3. If you have surveyed more than one hedge, please only fill the species list in once. Note that there are multiple sheets along the bottom for you to fill in the species present in each surveyed hedge.
4. You can save and continue at any time. Just hit the button at the bottom of the form.

Hedge survey

Please be aware that while only one dormouse survey needs to be completed, the 300 of hedgerow selected may require more than one hedgerow survey. If you need guidance on completing the hedgerow survey please see the PTES website https://hedgerowsurvey.ptes.org/survey-guidelines.
